HOW DO I GET STARTED WITH COSTCO HEALTH SOLUTIONS?
You can go online to access your specific pharmacy plan information through the Costco Health Solutions website.
Start your account:
- Call Costco Health Solutions Customer Care at 877.908.6024 to receive your unique ID and personal code.
- Register for an account at the Costco Health Solutions member portal.
- After entering your information, you will be redirected to a dedicated website to view your presecription records.
HOW DO I FIND AN IN-NETWORK PHARMACY?
You can use Costco's pharmacy locator or download their PDF directory to locate an in-network pharmacy near you.
HOW DO I DETERMINE IF A SPECIFIC DRUG IS COVERED AND HOW MUCH THE COPAY WILL BE?
Download your formularies below. They are updated monthly. The coverage associated with each drug is noted on the formulary. Also included is information regarding which drug products are subject to prior authorization and/or quantity limits. The formulary document does not list every covered medication.
The formulary does not specifically indicate the dollar amount members can expect to pay for each drug product. Your Schedule of Benefits will describe the different copay tiers based on these classes of medications. You are likely to pay less for generic or preferred brand name drugs than those in the non-preferred or specialty category. Talking with your doctor about your medication options can save you money.

HOW MUCH DO GENERIC MEDICATIONS COST?
Effective January 1, 2024, UnitedAg members can access Tier 1 generic drugs for $0 copay* at retail Costco Pharmacies.

*Important: Specialty generics are not included. This benefit does not apply at Costco Mail Order, Costco Specialty pharmacies or other in-network pharmacies. HSA plan members will pay $9 per 30-day supply. A $0 generic prescription fee will apply to HSA plan members after the annual deductible is met.
HOW DO I ENROLL IN THE COSTCO MAIL ORDER PHARMACY?
Costco's Mail Order Pharmacy program is designed for maintenance (long term) prescriptions and can help save on your overall cost and trips to the pharmacy. You can enroll in this program by calling Costco Pharmacy at 800.607.6861 or visit pharmacy.costco.com.
HOW DO I ENROLL IN COSTCO SPECIALTY SERVICES?
Costco Specialty Services provides unique, customized pharmacy services. You will be assigned a Patient Care Coordinator to initiate and coordinate your services to ensure one-call assistance throughout your course of therapy. You can enroll in this program by calling Costco Specialty Services at 866.443.0060 or visit costcohealthsolutions.com.

MY MEDICATION NEEDS A PRIOR AUTHORIZATION, BUT I NEED MY MEDICATION NOW, WHAT CAN I DO?
Typically, your doctor submits the prior authorization request on your behalf. Costco Health Solutions will review the prior authorization request within 48 hours of receiving complete information from your physician.
In the meantime, contact our Member Services team at 800.223.4590. They will assist you with a one-time exception override while your medications prior authorization is being processed through Costco Health Solutions.
